Oncology Mitra — We walk with you

User Platform and Informed Consent

A plain reading copy of the consent you are asked to give in the OM app. It separates account acceptance, permission for necessary data processing, and consent to a particular consultation.

User Platform and Informed Consent · Version 1.0 · Prepared 10 September 2026 · Effective 1 September 2026. Oncology Mitra is operated by ZTS Global Ventures Private Limited (CIN U62090UP2026PTC247240 · GSTIN 09AADCZ0004D1ZU), E-1190, E Block, Rajajipuram, Lucknow, Uttar Pradesh 226017.

This consent does not replace the Terms, the Privacy Notice, or a professional's discussion with you.

Oncology Mitra is not an emergency service.

Do not rely on it for urgent help or continuous monitoring. In immediate danger in India, call 112 or seek emergency care. Journals, mood entries, posts and notifications are not continuously reviewed crisis messages.

1. Eligibility and choice

This consent is for a person aged 18 or above who can make an informed decision. Being a patient or having a mental health condition does not by itself mean a person cannot decide. You may ask for an explanation, translation or other support. This form must not be used as a substitute for a properly established child or representative consent process.

You may choose whether to use a service, ask questions, stop a session or seek another provider. Withdrawing permission necessary to deliver a particular service may prevent that service from continuing, but declining optional marketing or publicity does not affect unrelated services. Any fee consequence must follow the terms disclosed before payment and applicable law.

2. What the platform provides

The platform facilitates provider discovery, bookings, memberships where offered, payments, audio/video consultations and supportive features such as nutrition plans, wellness content, mood logs, journals and community participation. Your available services depend on the actual offering shown in the app and your entitlement.

The provider's title, qualifications and exact scope must be shown or explained before the session. Counselling, clinical psychological services and general wellness have different scopes. Platform access is not a promise of diagnosis, a cure or a particular result. General wellness content, mood charts and journal prompts do not replace an individual clinical assessment or your treating team's advice.

The Company remains responsible for its platform obligations, and each provider for their professional acts within the applicable relationship. This consent does not exempt either from responsibility for negligence or breach of law.

3. Remote consultation and safety

Remote sessions may limit physical examination, observation and emergency assistance. Connectivity failures, interruptions and privacy risks can occur, and in-person care may be more appropriate. Use a private setting where possible, tell the provider if someone else is present and agree a reconnection plan. Camera and microphone permissions are used only as needed for the chosen service; a video session may not function without camera permission.

Recording is not authorised by accepting this consent. Any proposed recording, transcript or AI analysis requires a separately explained lawful process identifying purpose, recipients, retention and choices before it begins. Do not record another participant without appropriate permission.

Give your current location when reasonably requested for remote safety. Emergency contact details are optional; if suitability cannot be established safely, the professional should explain alternatives.

4. Service completion, plans and payment

Fees, included services, cancellation and no-show rules and refund conditions must be displayed before booking or purchase. Ask for clarification before paying. Connectivity failure or an incomplete service may require review under those terms and applicable law. See our Refund & Cancellation Policy.

A consultation-end code is used to record the platform's completion process. Share it only when the agreed session has actually concluded. Sharing the code does not mean you are satisfied, that the service succeeded, or that you waive a complaint, refund claim or other right. You may report an incorrect completion record.

Where a nutrition plan is included, the provider must explain when and how it will become available. A completed consultation may be followed by a preparation period. A plan should not be treated as delivered while it is shown as preparing.

5. Information and confidentiality

Necessary information may include your account and profile details, booking and payment references, information you choose to share for the consultation, relevant professional records and technical connection data. The Privacy Notice specifies the actual categories, purposes, recipients, vendors and retention.

Your professional should explain confidentiality and its limited lawful exceptions, including disclosures required by law or justified emergency circumstances. Relatives or caregivers do not automatically receive access. Any permitted disclosure should be proportionate. Do not enter another person's private information without an appropriate basis.

Journals and mood entries are personal features; they are not automatically shared with every provider through this consent. Public community content can be visible to other users and may be copied. Read the community rules before posting and avoid publishing confidential medical information. Account deletion does not automatically erase records that must be retained for a lawful reason.

6. Account choices

When you accept in the app, you confirm each of the following separately:

Necessary transaction and safety communications relating to a requested service may still be sent through an appropriate disclosed channel. This does not permit unrelated advertising.

7. Consent for a particular consultation

Before each consultation, the provider's name, represented title and scope, the service and its purpose, the booking reference, fee and refund terms are recorded. The provider must have explained the proposed service, foreseeable risks, limitations, benefits without guarantees, alternatives and confidentiality limits, and answered your questions. You consent voluntarily to that remote consultation and the necessary use of the information you share for it, and you may pause or withdraw. Any support person or observer present is identified and agreed.

A checkbox does not replace this discussion. The provider records that suitability and decision-making support needs were assessed and the above explained in an understandable manner.

8. Optional choices

Optional reminders and marketing by push notification, SMS or email start unchecked and can be changed independently. Community participation follows our Community Guidelines. Media, testimonials and webinar recordings require separate forms; no such permission is granted through this consent. No AI companion consent is collected here — if such a service is introduced, its operation, data processing and risks will be separately explained before use.

9. Withdrawal, questions and complaints

Withdraw consent or ask about your choices through an available feature control or by emailing support@oncologymitra.com. The email route remains available even if a particular in-app control is not implemented. We must explain the effect on the requested service, stop consent-based processing as required by law and identify any separate lawful retention obligation. Prior lawful processing is not retrospectively invalidated. Fresh or materially changed services may require new consent.

You may raise a complaint without losing non-waivable rights. Correspondence address: E-1190, E Block, Rajajipuram, Lucknow, Uttar Pradesh 226017. Our designated Grievance Officer is Mansi Dham +91 78807 62277, support@oncologymitra.com. See Grievance Redressal for the full process.
